Please be advised that from 31st December 2023 My Health Online will no longer be available.
To successfully access GP services online via the NHS Wales App, patients must:
- have a fully verified NHS login, and will require a valid photo ID (passport, driving licence or national ID card) to set one up
- an email address that isn’t shared (it must be unique to that person)
- be aged 16 or over
